If your child has a diagnosed health condition and requires specific care needs or has a medical condition, the school will work with parents to ensure the inclusion and safe particpation of all students in educational experiences.

Parents/Guardians will need to provide the school with-

- Infomation regarding the medial condition

- Up to date medical infomation on the students Operoo profile

- ASCIA Action Plan for Asthma, Anaphyalaxis, Allergy (if applicable)

- MACS Medication Authority Form (Apr 2024)- if applicable

- MACS Medical Management Plan (Apr 2024)- if applicable

The school will provide parents with-

- MACS Student Health Support Plan (Apr 2024)

- MACS Medical Management Communication Plan (Apr 2024)
